精品论文 参考文献 糖尿病患者糖化血红蛋白检测的临床意义及其质量控制 凉山州中西医结合医院 615000 摘要：目的：探讨糖化血红蛋白监测在糖尿病患者治疗疗效评估中的应用及其结果的准确性和稳定性。方法：测定健康组及糖尿病组的FPG、糖耐量测定并采用免疫凝集法、色谱法分别测定两组HbAlc，比较不同时间两种方法测定HbAlc的结果有无统计学差异，并分析时间因素对检测结果的稳定性的影响。结果：健康组HbA1c阳性率明显低于全血FPG、糖筛查实验阳性率，相比而言糖尿病组HbA1c阳性率与全血FPG、糖筛查实验阳性率无明显差异；而健康组组间比较：免疫组患者的FPG、糖筛查实验、HbAlc与健康组无明显差异（P＞0.05）。健康组组内比较：免疫组HbA1c阳性率明显低于FPG及糖筛查阳性率（P＜0.05）；色谱组HbA1c阳性率和FPG及糖筛查阳性率无明显差异（P＞0.05）；糖尿病组组间比较：免疫组患者的FPG、糖筛查实验、HbAlc与健康组无明显差异（P＞0.05）。糖尿病组组内比较：免疫组HbA1c阳性率明显低于FPG及糖筛查阳性率（P＜0.05）；色谱组HbA1c阳性率和FPG及糖筛查阳性率无明显差异（P＞0.05）；并且糖尿病组全血标本在4℃条件下放置2h、1d、3d、5d、1w、2w、1m、6m后使用免疫法、色谱法检验结果显示影响较小。结论：糖尿病患者或葡萄糖耐量异常时，联合检查患者的空腹血糖与HbA1c，对于判断患者的血糖水平变化及糖尿病或葡萄糖耐量异常的预防、诊断与治疗均具有重要的临床意义。 关键词：糖化血红蛋白；糖尿病；临床意义；质量控制 [Abstract] Objective：To explore the application of HbAlc monitor in the efficacy assessments of diabeticrsquo;s treatment and the accuracy and stability of the results. Methods：determining the the FPG and glucose tolerance of health group and diabetic group，and measuring their HbAlc by using immune agglutination test and chromatography，comparing the result of HbAlc determined by those two methods in different time，and see there is statistical difference or not. And analyzing time factorrsquo;s influence. Results：the positive rate of healthy grouprsquo;s HbAlc was significantly lower than the positive rate of blood FPG and glucose screening test. On the contrary，there was no much difference between positive rate of HbA1c of diabetic group and positive rate of blood FPG and glucose screening test. Compared between the healthy groups，there was no significant difference between the results of FPG，glucose screening test，HbAlc in immune group and that of health group（P gt; 0.05）. In the healthy group：positive rate of HbAlc in immune group was significantly lower than that of FPG and glucose screening（P lt; 0.05）；There was no significant difference in the positive rate of HbA1c and the positive rate of FPG and glucose screening test in the chromatograph group（P gt; 0.05）.
