冠心病合并糖尿病患者冠状动脉旁路移植术围手术期处理.doc

【摘要】 　　目的：研究伴冠心病合并糖尿病患者冠状动脉旁路移植术（C A B G ） 围手术期的处理。方法： 以2000 年1月至2006 年5 月的189 例冠心病行C A B G 患者为研究对象，按是否同时合并有糖尿病分为糖尿病组（56 例）和无糖尿病组（133 例），对比两组临床特点，搭桥支数，术后处理及术后并发症。结果： 冠心病合并糖尿病患者冠脉病变广泛而严重（P ＜ 0101 ），搭桥4 支的比例明显高于非糖尿病组（5613 %∶19163 %， P ＜ 0101 ）），术后并发症的发生率明显高于非糖尿病例组（ P ＜ 0101 ）。结论：冠心病合并糖尿病患者较无糖尿病患者冠脉病变更加严重，合并症更多，需认真、积极对待。 【关键词】 冠状动脉疾病；冠状动脉旁路移植术；糖尿病 　　 Treatment of peri2surgery period of coronary artery bypass graftin patient with coronary artery disease com plicating diabe2tes 　　Z H A O Shun2ying ，C H E N Yong2an ， W A N G Qiong 　　Chinese Journal of Cardiovascular Rehabilitation M edicine ，2007 ，16 （4 ）：401 Abstract ：Objective ：To study the treatment of peri2surgery period of coronary artery bypass graftin patient with cor2onary artery disease （C A D ）co m plicating diabetes .M ethods ：A total of 189 C A D patients were divided two groupsaccording isnt co m plicating diabetes ：C A D patients co m plicating diabetes group （observe group ，56 cases ）and C A Dpatients without diabetes group （control group ，133 cases ）.The data of two groups were com pared .Results ：Com2pared with control group the lesion of coronary artery was more extensive and severity （P ＜ 0101 ），the patient with 4 branches of bypass graft was more（P ＜ 0101 ），the com plication increased （P ＜ 0101 ）in observe group .Conclusion ： Com pared with C A D patients without diabetes ，the lesion of coronary artery is more extensive and severity and the com plication is more in C A D patients with diabetes ，it m ust be paid attention . Key words ：Coronary artery disease ；Coronary artery bypass graft；Diabetes 　　糖尿病是冠心病（C H D ） 的独立危险因素［1 ］。多数冠心病合并糖尿病患者冠脉病变严重，治疗难度增加。我院自2000 年1 月至2006 年6 月共进行冠脉旁路移植术（C A B G ）189 例，其中冠心病伴有糖尿病的患者有56 例，近期效果满意，报告如下： 1 　资料与方法 1.1 　一般资料 选择189 例冠心病行C A B G 病人为分析对象，其中合并有2 型糖尿病者56 例被设为糖尿病组（A组），男34 例， 女为22 例、年龄45 ， 79 ， 平均（5413 ±811 ）岁，另133 例为非糖尿病组（B 组），男89 例， 女44 例， 年龄46 ， 80 ， 平均（5612 ±1111 ）岁。两组患者冠心病和2 型糖尿病的诊断均符合W H O 关于冠心病和2 型糖尿病的诊断标准，两组临床特点及冠脉造影比较见
