无创呼吸机治疗慢性阻塞性肺疾病合并呼吸衰竭临床观察
【摘要】 目的:观察无创呼吸机治疗慢性阻塞性肺疾病(COPD)急性加重期合并呼吸衰竭的患者纠正缺氧和改善二氧化碳潴留的临床疗效。方法:选取符合标准的60例慢性阻塞性肺疾病急性加重期合并呼吸衰竭患者为研究对象,对照组20例鼻导管吸氧,研究1组20例COPD急性加重期合并Ⅰ型呼吸衰竭患者无创呼吸机氧疗;研究2组20例COPD急性加重期合并Ⅱ型呼吸衰竭患者无创呼吸机氧疗。观察氧疗前后的PaO2、PaCO2水平。结果:研究1组和研究2组的PaO2经过治疗后明显提高,研究2组PaCO2经过治疗后明显降低,差异均有统计学意义(P0.05)。结论:无创呼吸机能有效的纠正慢阻肺急性加重期合并呼吸衰竭患者的缺氧和二氧化碳潴留。

【Abstract】 Objective:To correct hypoxia and improve the clinical curative effect of carbon dioxide retention of noninvasive ventilator therapy on chronic obstructive pulmonary disease in patients with acute aggravating period of combination of respiratory failure.Method:60 cases of chronic obstructive pulmonary disease in patients with acute aggravating period combination of respiratory failure were selected as the research objects, 20 cases when administering oxygen inhalation through nasal catheter were selected as the control group, 20 cases of COPD exacerbations merger Ⅰ type of respiratory failure patients noninvasive ventilator oxygen therapy were set as research phase 1; 20 cases of COPD exacerbations period merger Ⅱ type of respiratory failure patients noninvasive ventilator oxygen therapy were set as research phase 2. PaO2, PaCO2 level were observed before and after oxygen cure.Result:1 set of PaO2 and 2 set of PaO2 improved obviously after treatment, the 2 set after treatment PaCO2 decrease were statistically significant (P0.05).Conclusion:Noninvasive respiratory function effective correction of COPD patients with acute aggravating period combination of respiratory failure of oxygen and carbon dioxide retention.
