Drugs and Clinic 药物与临床 PIVAS肠外营养不合理医嘱干预情况及防范措施 白凤芝，孙录*，王华*，于倩 吉林大学中日联谊医院药品管理部，吉林长春 130033 [摘要] 目的：探讨我院静脉用药调配中心(PIVAS)肠外营养医嘱的合理性及药师干预的实施效果及防范措施。方法：选取我院PIVAS2013年 肠外营养医嘱47740组及2014年肠外营养医嘱59269组中药师审核的不合理医嘱作对比分析，通过有效沟通、处方点评、查房会诊、药学监 测及药学经济学的研究等方法进行干预。结果：干预后不合理医嘱显著下降，由2013年的2463条(5.16％)下降到2014年的1297条(2.19％)。 结论：PIVAS肠外营养不合理医嘱干预有效、可行，干预效果明显，提高了静脉用药的安全性与合理性。 [关键词] 静脉用药调配中心；不合理医嘱；临床药师；干预 [中图分类号] R969.3　　[文献标识码]　A　　[文章编号]　1672-2809(2016)02-0041-03　 The Intervention and Prevention Measures for the Irrational Prescription of PNT in PIVAS BAI Feng-zhi, SUN Lu*, WANG Hua*,YU Qian Department of Drug administration, China-Japan Union Hospital of Jilin University, Changchun Jilin 130033, China [Abstract] Objective: To discuss the rationality, the implementation of the pharmacists intervention effect and measures to prevent of parenteral nutrition prescriptions of pharmacy intravenous admixture services(PIVAS) in our hospital. Methods: 47740 prescriptions in 2013 and 59269 prescriptions in 2014 were selected; all of the irrational prescriptions in PIVAS were compared and analyzed by pharmacists. The methods were taken to intervened prescriptions, including effective communication, prescription evaluation, consultation rounds, pharmaceutical care and pharmacy economic analysis. Results: The irrational prescriptions declined signi?cantly from 2463(5.16%) in 2013 to 1297(2.19%) in 2014 after interventions. Conclusion: The interventions of irrational prescriptions in PIVAS were effective, feasible and the effect of intervention was obvious to improve the safety and rationality of pharmacy intravenous.
