药物与临床 Drugs and Clinic PIVAS审方药师对妇科不合理用药医嘱干预分析 江楠，张洪峰，王乐，魏亚超，李晓翠，陈晨 河北省邯郸市中心医院 药学部，056001 [摘要] 目的：统计并分析我院静脉药物配置中心接收妇一科的不合理医嘱，为药师提供审方参考，也为妇科临床的合理用药 提供依据。方法：依据《新编药物学》、《药物临床信息参考》、《临床静脉用药调配与配伍速查》、《432种静脉注射剂配 伍指南》及药品说明书等统计并分析我院妇一科2014年10月～2015年3 月98份静脉药物配置中心接受的不合理医嘱。结果：妇 一科两个季度18495份处方中共包含不合理医嘱98份，占医嘱总数的0.53% 。所涉及的不合理医嘱处方内容主要包括药物选择 不适宜、用法用量不适宜、给药途径不适宜、联合用药不适宜、重复给药、有配伍禁忌或不良相互作用、溶媒不适宜、医嘱 录入错误等问题。结论：审方药师在合理用药过程中发挥着重要作用，通过审方药师干预不合理处方，可进一步确保妇科的 用药安全。 [关键词] 静脉药物配置；审方药师；妇科；不合理医嘱 [中图分类号] R979.1　　[文献标识码]　A　　[文章编号]　1672-2809(2015)18-0025-04 PIVAS Review Prescription Pharmacist Intervention Analysis for the Gynecological Unreasonable Medication orders JIANG Nan, ZHANG Hong-feng, WANG Le, WEI Ya-chao, LI Xiao-cui, CHEN chen Department of pharmacy, Hebei Handan Central Hospital ，056001 [Abstract] Objective: To analyze the gynecological unreasonable prescription of pharmacy intravenous admixture services (PIVAS) received in our hospital, and to provide reference and the basis for pharmacists and clinical gynecology rational drug use respectively. Method: The irrational prescription of PIVAS received from October 2014 to March 2015 were statistically classified and analyzed according to New Pharmacology, Clinical Drug Information Reference, Clinical intravenous drug use Quick deployment and compatibility, 432 kinds of intravenous injections Compatibility Guide and the package inserts of drug. Results: There were 98 irrational prescriptions (accounted for 0.53%) in 18495 prescriptions. The irrational prescriptions were found, including drug selecting, dosage, route, combination therapy, repeated administration, incompatibility or adverse interactions, solvent, physician order entry errors and other issues. Conclusion: The review prescription pharmacists play an important role in rational drug use.
