腹腔镜子宫肌瘤剔除术两种缝合的方法疗效比较 　　[摘要] 目的 探讨腹腔镜子宫肌瘤剔除术常规连续缝合和赵氏法缝合的临床有效性与安全性。方法 方便选取2014年1月―2016年12月该院收治的102例腹腔镜子宫肌瘤剔除患者为研究对象，依缝合方法不同分实验组（赵氏法缝合，51例）和对照组（常规缝合，51例）。对比观察两组基本治疗指标。结果 实验组手术时间（54.1±11.5）min，术中出血量（39.3±12.2）mL，术后引流时间（28.7±3.1）h，Hb下降幅度（4.9±2.0）g/L，术后住院时间（3.4±0.4）d，术后并发症发生率0.00%，各项指标均优于对照组（P0.05）。结论 赵氏腹腔镜下肌瘤假包膜套扎连续缝合治疗子宫肌瘤效果确切，可有效减少出血，预防相关并发症，促进患者早期康复，应用安全有效，值得临床推广。 　　[关键词] 腹腔镜；子宫肌瘤剔除术；假包膜套扎；赵氏缝合 　　[中图分类号] R737.6 [文献标识码] A [文章编号] 1674-0742（2017）09（b）-0068-03 　　[Abstract] Objective This paper tries to investigate the clinical efficacy and safety of conventional continuous suture and Zhao’s suture for laparoscopic myomectomy. Methods The 102 cases of laparoscopic myomectomy in this hospital from January 2014 to December 2016 were convenient selection and divided into the experimental groups（Zhao’s suture， 51 cases） and the control group（conventional suture， 51 cases） by different suture methods. The basic therapeutic indexes of two groups were observed. Results Operation time of the experimental group was（54.1±11.5）min， intraoperative bleeding was （39.3±12.2）mL， postoperative drainage time was（28.7±3.1）h， Hb decrease amplitude was（4.9±2.0）g/L， postoperative hospitalization time was （3.4±0.4）days， postoperative complication rate was 0.00%， all indexes were better than those in the control group（P0.05）. Conclusion The Zhao’s suture treatment for laparoscopic myomectomy is effective in uterine fibroids， which can effectively reduce the bleeding， prevent related complications， promote early rehabilitation of patients， with safe and effective application， so it is worthy of clinical promotion. 　　[Key words] Laparoscope； Excision of hysteromyoma； False envelope ligation； Zhao’s suture 　　目前，?R床可行子宫肌瘤手术方法及入路有多种。但是，子宫肌瘤血运丰富，切除后如创口缝合不当、止血不佳，则易引起残腔血肿、粘连等并发症，影响疗效及预后[1]。为解决上述问题，提高止血效果，保证手术安全性，中国医科大学附属盛京医院赵福杰教授设计了垂体后叶素结合腹腔镜下肌瘤假包膜套扎及镜下连续缝合技术[2]。该院近年来以此治疗子宫肌瘤取得良好疗效，文章现以2014年1月―2016年12月该院收治的102例子宫肌瘤患者为例进行分析和探讨，现报道如下。 　　1 资料与方法 　　1.1 一般资料 　　方便选取该院收治的102例子宫肌瘤患者为研究对象。纳入标准：①B超检查明确
