Methods: All the irrational TPN medical orders of PIVAS from Jan. 2012 to Dec. 2012 were checked, classi? ed and statistically analyzed. Results: The main problems of the irrational TPN medical orders were the drug composition, solvent, ion concentration, administration dosage, ? uid volume, repeated administration, incompatibility, adverse drug interactions and the energy matching, etc. Conclusion: Pharmacists should provide reliable information to the clinic, communicate with the clinic through contact letters, so as to make contributions to ensure the rational and safe use of drugs.
