血清Hcy和CysC联合检测在高血压早期肾病诊断中应用分析
[摘要] 目的 探讨血清同型半胱氨酸(Hcy)与胱抑素C(CysC)联合检测在高血压早期肾病诊断中的应用价值。 方法 方便?x取2016年4月―2017年7月在该院就诊的高血压患者70例,根据肾小球滤过率不同分为对照组(高血压,22例)和观察组(高血压早期肾病,48例),均行血清Hcy、CysC、肌酐(Cr)含量检测,并比较两组血清Hcy、CysC、Cr水平。结果 观察组血清Hcy、CysC、Cr水平[(16.86±5.31)μmol/L、(1.38±0.45)mg/L、(78.94±25.47)μmol/L]均明显高于对照组[(11.92±3.47)μmol/L、(0.89±0.19)mg/L、(53.46±7.67)μmol/L],差异有统计学意义(P0.05);血清Hcy联合CysC检测的阳性检出率(89.58%)明显高于单独血清Hcy、CysC、Cr水平检测(64.58%、68.75%、4.17%),差异有统计学意义(P0.05)。结论 血清Hcy与CysC联合检测可有效检出高血压早期肾病,为后期治疗方案的制定提供依据。

[Abstract] Objective This paper tries to study the application value of joint detection of homocystein (Hcy) and cystatin C (CysC) in the diagnosis of early hypertensive nephropathy (HN). Methods 70 hypertension patients from April 2016 to July 2017 were divided into the control group (hypertension, 22 cases) and the observation group (early EN, 48 cases) by glomerular filtration rate. They were given detection of Hcy, CysC, creatinine (Cr) content. The levels of Hcy, CysC, Cr of two groups were compared. Results The levels of Hcy, CysC, Cr in observation group [(16.86±5.31)μmol/L, (1.38±0.45)mg/L, (78.94±25.47)μmol/L] were higher than control group [(11.92±3.47)μmol/L, (0.89±0.19)mg/L, (53.46±7.67)μmol/L] (P0.05); The positive detection rate of joint detection of Hcy and CysC(89.58%) was higher than that of single detection of Hcy, CysC, Cr(64.58%, 68.75%, 4.17%) and the difference was significant (P0.05). Conclusion Joint detection of Hcy and CysC can detect early HN and provide basis for treatment regimen.

高血压肾病是一种较为严重的高血压并发症,其发生率高达20.87%,若不能及时诊断并采取措施进行治疗,将进展为终末期肾病,甚至造成患者死亡[1]。因此,及时发现高血压早期肾病,对改善患者预后有着较为重要的作用。肾穿刺是早期诊断高血压肾病的金标准,但因医疗条件、患者及家属对高血压肾病认知程度等因素的影响,将限制其推广[2]。目前,肾小球滤过率是评估肾脏功能的最好指标,主要通过血清Cr对肾小球滤过率进行估算,但其易受年龄、肌肉等因素的影响[3]。基于此,该研究对2016年4月―2017年7月该院收治的70例高血压患者进行究,旨在探讨血清Hcy与CysC联合检测在高血压早期肾病诊断中的应用价值,现报道如下。
