老年医学与保健 2017 年第 23 卷第 1 期 Geriatr Health Care ，2017, Vol. 23.No. 1 体质指数对老年乳腺癌新辅助内分泌治疗功效的影响 葛睿，张凌捷，胡天华，李洁静，唐文皓，程爱群 复旦大学附属华东医院普外科，上海 200040 [摘要] 目的研究体质指数（ body mass index ，B M I ) 对激素受体阳性老年乳腺癌患者的新辅助内分泌治疗（NET) 功效的影响。方法回顾性分析研究自 1995年 1 月 1 日一2014 年 1 2 月 3 1 日在复旦大学附属华东医院诊治的年龄 6 5 岁，并且接受3 个月以上 N E T 的 107例激素受体阳性的老年乳腺癌患者临床资料。根据 B M I 将其分为高B M I 组和低/ 标准 B M I 组。采用卡方检验分析和比较治疗后2 组病理完全缓解率（pCR) ，客观缓解率（O R ) 的差异，并运用Kaplan- M eier 法和 log-rank 捡验比较无复发生存（R F S ) 和乳腺癌特异性生存（B C S S ) 的差异。结果中位随访 6 .8 年 （区间为 1.7 〜1 0 .1年 ），低/ 标准 B M I 组患者治疗后O R 率优于高B M I 组 （O R 率 64.2% v y 4 7 .5 % ，戶 0 .0 5 ) ，2 组患者的pCR 率 （7.5% ra 7 .5 % ，P = 0 .7 9 )， RFS (48.4% 朽 4 1 .1 % ，H R 9 5 % C I0.93 (0 .4 5 -1 .9 6 ) ，P = 0 .8 6 ) 和 BCSS (74.0% ra 59.3% H R 9 5 % C I0.74 (0.26 〜2.07) ，P = 0 .5 6 ) 均无统计学差异。结 论 B M I 对激素受体阳性老年乳腺癌患者内分泌治 疗的短期疗效有一定影响，但对于长期生存的影响不明显。 戌键词 ] 体质指数；新辅助内分泌治疗；队列研究 Impact of BMI on Curative Effect of Neoadjuvant Endocrine Therapy in Elderly Breast Cancer Patients Ge Rui， Zhang Lingjie， Hu Tianhua， Li Jiejing， Tang Wenhao, Cheng Aiqun 氺 Department o f Surgery, Huadong Hospital Affiliated to Fudan University, Shanghai, 200040, P.
