高强度聚焦超声治疗子宫（腺）肌瘤血尿原因分析 扬州市妇幼保健院超声科 周凯 申凤珠 左玲 【摘要】目的 探寻高强度聚焦超声治疗子宫（腺）肌瘤发生血尿的原因。方法 1.回顾性分析2006.12.-2008.11.出现血尿的所有病例。2. 2008.12.-2009.4.选择40对患者分为两组：甲组插导尿管，乙组不插导尿管。治疗时一一对应的两患者的治疗参数相同，比较两组的血尿发生率。结果 1. 2006.12.-2008.11. 共有91例出现血尿，治疗过程中，膀胱内均出现剧烈喷雾现象。2. 甲组血尿发生率55%，乙组为0。结论 血尿与插导尿管的关联性明确。工作中，为保证治疗效果,减少血尿的发生，应尽量不插导尿管。 【关键词】 高强度聚焦超声 血尿 子宫肌瘤 Analysis of high intensity focused ultrasound treatment for hematuria in hysteromyoma (Adenomyoma) ZHOU Kai，SHEN Feng-zhu，ZUO Ling Maternal and Child Care Center Of Yangzhou，Jiangsu province 225002，China 【Abstract】 Objective To explore the causes of hematuria in hysteromyoma (Adenomyoma) by high intensity focused ultrasound（HIFU）. Methods 1. A retrospective analysis was made of all the cases collected between Dec. 2006 and Nov. 2008. 2. 40 pairs of patients chosen between Dec. 2008 and Apr. 2009 were classified into two groups: Group A inserted with urinary catheters while Group B with none. Treatment parameters of each pair were the same and the incidence of hematuria was then compared between the two groups. Results 1. There were in total 91 cases of hematuria from Dec. 2006 to Nov. 2008 and all of them saw intense spray in the bladder. 2. The incidence of hematuria was 75% in Group A and it was 0 in Group B. Conclusion The correlation is clear between hematuria and catheter insertion. Therefore, to ensure treatment effect and hematuria reduction, catheter insertion should be avoided if possible. 【Key words】 HIFU；Hematuria ;Hysteromyoma 血尿是高强度聚焦超声（HIFU）治疗子宫（腺）肌瘤并发症之一，确切原因尚未明确。治疗过程中，因出现血尿而被迫终止治疗的情况并不少见。血尿的发生不仅影响了治疗还给患者带来不必要的损伤和心理负担，同时也使患者对治疗本身产生怀疑，因此找到血尿的原因并采取措施避免之对医患双方均十分必要。 资料与方法 一般资料 1.1研究对象：2006年12月—2009年4月在我院超声刀治疗中心接受治疗的子宫（腺）肌瘤患者，年龄23—51岁，肌瘤直径18mm—62mm，治疗次数1—3次。 1.2治疗前准备：饮水憋尿、腹部备皮、酒精脱脂、治疗前半小时肌注杜冷丁50mg。肝、肾功能，血、尿常规均正常。 2006年12月—2008年11月共527例治疗前均插导尿管； 2008年12月—2009年4月共103例，部分插尿管，部分未插。 1.3 HIFU治疗系统及治疗方法 HIFU- NIT9000肿瘤消融机（上海爱申科技发展有限公司）。治疗参数：治疗头3-5个、点治疗时间200ms-300ms、点治疗次数8次、单次治疗时长60min-90min。单次治疗包括两个过程：逐层治疗和间断加强治疗。 研究方法 2.1 回顾性分
