中华创伤杂志2002 年 6 月第 18 卷第 6 期 　Chin J Traumatol , June 2002 , Vol. 18 , No. 6 3·61 · ·论著 · 肾脏损伤分型在诊治中的作用 张唯力 　吴刚　杨光永 　胡自力 　董万 　徐光勇 　刘川 　乔天愚 　　【摘要】　目的　探讨肾脏损伤分型在诊治中的作用 。　方法 　回顾分析 自 1990 年 8 月至 2001 年 2 月收治的98 例肾脏损伤资料 ,其中男 78 例 ,女 20 例 ;年龄 11～80 岁 ,其中 40 岁 66 例 , 占67. 3 % 。本组挫伤 60 例 ,血肿 16 例 ,裂伤 6 例 ,粉碎伤 16 例 ; 以直接打击伤 、坠落伤和交通伤为 ( ) ( ) ( ) 多 ,分别为 50 例 51. 0 % 、27 例 27. 6 % 和 14 例 14. 3 % ; 血尿 96 例 , 其 中肉眼血尿 52 例 ( ) ( ) 53. 1 % ,镜下血尿 44 例 44. 9 % 。本组均按美国创伤外科学会器官损伤标准委员会制定的肾脏 损伤标准分型 。　结果 　 Ⅰ型60 例 , Ⅱ型 16 例 , Ⅲ型3 例 , Ⅳ型3 例 , Ⅴ型 16 例 。 Ⅰ～ Ⅲ型肾脏损 伤均为保守治疗 , Ⅳ型行肾脏修补术 , Ⅴ型行肾脏切除术 。98 例全部治愈 。　结论 　 肾脏损伤分 型与患者的年龄 、致伤原因、合并伤 、血尿程度以及治疗方式均有关 ,临床可根据分型情况指导治 疗 。 【关键词】　肾; 　创伤和损伤 ; 　诊断 ; 　治疗 Role of injury classification in diagnosis and treatment of renal injury 　　ZHANG Weili , WU Gang , YANG Guangyong , et al .
