钽金属Jumbo杯在PaproskyⅡ及Ⅲ型髋臼缺损髋关节翻修中的应用 古凌川杨柳何锐 第三军医大学西南医院关节外科 摘要?目的 探讨应用钽金属Jumbo杯对Paprosky型及以上严重髋臼骨缺损进行重建的疗效观察。方法?回顾性分析2009年月至201年月在髋术中使用钽金属Jumbo杯重建髋臼骨缺损的患者例的病例资料,男例,女例;年龄76岁,平均(5.8±15.3)岁。Paprosky分型Ⅱb型例,Ⅱc型例,a型例。观察术后髋臼假体、生存率、并发症及Harris髋关节评分。结果?随访时间-54月,平均.7月。末次随访时Harris评分:Paprosky Ⅱ型为.8±11.8分,高于术前.3±12.5分Ⅲ型7.8±9.3分,高于术前±10.6分差异有统计学意义。随访期间钽金属臼杯固定良好,未出现松动及移位,假体生存率100%。按照Anderson骨长入评价标准,髋臼杯均出现不同程度骨长入透光线,1例于术后4周出现后脱位。结论?钽金属臼杯重建paproskyⅡ型及以上严重髋臼骨缺损,能多孔结构利于骨长入,期疗效好。400038,重庆,第三军医大学西南医院关节外科 通信作者:杨柳,E-mail:jointsurgery@163.com Reconstruction of acetabular bone loss with porous tantalum jumbo acetabular components in revision hip arthroplasty GU Lingchuan, CHEN Guangxing,YANG Liu, HE Rui, PENG Yang,YANG Pengfei.Center for Joint Surgery,Southwest Hospital,the Third Military Medical University,Chongqing 400038, China Abstract] Objective To explore the short-term efficacy of the reconstruction of acetabular bone loss with porous tantalum jumbo acetabular components in revision hip arthroplasty.Methods 18 patients (18 hips)were included in this study,which had an acetabular replacement with tantalum jumbo acetabular components from September 2009 to September 2012.There were 8 males and 10 females,with the mean age of 56.8±15.3years(range from 37 to 76 years).The acetabular bone defects were classified as Paprosky classification,which included typeⅡb in 6,Ⅱc in 7,Ⅲa in 2 and Ⅲb in 3 cases.Data on prosthesis position, stability,survivorship,complications and Harris hip score were analyzed. Results The mean follow-up time was 35.7 months (range,24-54 months).In Paprosky Ⅱ,the Harris hip postoperative score(84.8±11.8)was significant higher than the preoperative score(48.3±12.5).In PaproskyⅢ,the Harris hip postoperative score(77.8±9.3)was significant higher than the preoperative score(38.5±10.6).All the acetabular components were well fixed by the radiograph imaging.No one had radiographic evidence of loosening, migration, radiolucent lines and osteolysis.Bone ingrowths was obvio
