鞘内注射丙戊茶碱对神经痛大鼠脊髓背角星形胶质细胞的影响 卜晓萱 李琼 沈志忠 (苏州大学附属第三人民医院 江苏 常州 213003) [摘要] 目的 观察鞘内注射丙戊茶碱(PPT)对慢性坐骨神经挤压损伤(CCI)大鼠脊髓背角星形胶质细胞(AST)的影响。 方法 雄性SD大鼠24只,随机分为三组(n =8):假手术组(A组);CCI组(B组);丙戊茶碱组(C组)。C组于术后3、5、7、9、11、13天鞘内注射丙戊茶碱10μg/rat,A、B组注射等量的生理盐水。用热辐射抬足法测定痛阈,用免疫组织化学方法测定脊髓背角AST的GFAP活性。 结果 与A组相比,B、C组术后第8和14天术侧热痛阈值下降,且GFAP的免疫反应阳性产物增多(p0.05);与B组相比,C组术后第8和14天术侧热痛阈值升高,GFAP的免疫反应阳性产物明显降低(p0.05)。 结论 慢性神经痛的机制可能涉及脊髓AST 的激活,丙戊茶碱通过抑制AST的激活而具有镇痛作用。 [关键词] 丙戊茶碱; 神经痛; 星形胶质细胞 Effect of intrathecal propentofylline on the astrocyte in the spinal dorsal root in the rats of chronic neuropathic pain Bu Xiaoxuan , Li Qiong, Shen Zhizhong. Department of Anesthesiology, Affiliated Third Hospital, Suzhou University, Changzhou 213003 CHINA [Abstract] Objective To investigate the effect of intrathecal propentofylline on the astrocyte in the spinal dorsal root in the rats of chronic neuropathic pain produced by chronic constrictive injury(CCI). Methods Twenty-four male SD rats randomly divided into 3 groups(n=8):A sham-operated group; B CCI group and C PPT group. C group was intrathecal PPT by passing a PE-10 catheter which was inserted into L4-5 spinal subarachnoid space at days 3、5、7、9、11 and 13 postsurgery. In group A and B normal saline was given intrathecally instead of PPT. The pain threshold was measured using the conventoined paw withdrawal test. The changes of the spinal astrocytes response was examined by immunohistochemistry of glial fibrillary acidic protein (GFAP) . Results In group B、C pain threshold to heat noxious stimulation decreased as compared to shame-operated group(A), and the spinal astrocytic activity was significantly elevated(p0.05). To compare with B, the pain threshold of C group increased as measured by behavioral test and the elevate in spinal GFAP immunostaining was also greatly blocked(p0.05). Conclusion The activity of the ASTs in spinal cord may be involved in the mechanism of chronic neuropathic pain. Propentofylline may prevent and treat the chronic neurop
