摘? 要:目的:探讨Ⅱ型糖尿病血糖控制欠佳时,糖化血红蛋白(GHb)与Lp(a)浓度的关系。方法:采用免疫浊度法对20例GHb(HbA1c)高于正常的Ⅱ型糖尿病患者和20例正常对照组的血浆Lp(a)进行测定。结果:GHb升高组和正常对照组的Lp(a)分布均呈偏态;GHb升高组Lp(a)显著高于对照组(p0.01);根据HbA1c水平,将GHb升高组分成三组,三组间Lp(a)的水平差异无统计学意义(p0.05);相关分析发现Lp(a)与HbA1c无相关性(p0.05)。结论:Ⅱ型糖尿病GHb的升高对Lp(a)水平无影响。

The? Research? of? Relationship? between? Glycosylated? Hemodlobin? and? Lipoprotein(a)? Levels? in? Diabetic? Patients
Abstract:Objective: To investigate the relationship between glycosylated hemodlobin (GHb) and lipoprotein(a) concentrations of poorly glycemic control in type 2 diabetic patients. Method: The plasma Lp(a) was analysed in highly GHb(HbA1c) levels in type 2 diabetic patients and 20 control subjects. Result: The distribution of plasma Lp(a) was skewed in highly glycated hemoglobin group and control subjects. The plasma Lp(a) levels in highly glycated hemoglobin group were significantly higher than those in control subjects (p0.01). Type 2 diabetic patients were divided into three groups based on GHb(HbA1c) levels. There were no significant differences in Lp(a) levels among three groups of diabetic patients(p0.05). Lp(a) levels were not correlated to HbA1c(p0.05). Conclusion:? Lp(a) levels were not influence in GHb rise in type 2 diabetic patients.

??? 目前已普遍公认,Lp(a)是导致心脑血管疾病的一个独立危险因子,Ⅱ型糖尿病的增加也导致心脑血管病的增加,在血糖控制欠佳的情况下,Lp(a)的水平是否与Ⅱ型糖尿病患者显著增加的心脑血管病有关、是否与GHb的水平有关,目前尚无定论。本文旨在探讨糖尿病患者GHb升高时,Lp(a)的水平及其关系。
1? 对象与方法
1.1? 研究对象
1.1.1? GHb升高组:在临床诊断为Ⅱ型糖尿病的患者中,选取HbA1c5.8者20例。年龄为42~75岁,其中男为12例,女为8例。按GHb水平分为3组:组1 HbA1c5.8~8%,,共7例;组2 HbA1c8~11%,共8例;组3 HbA1c11%,共5例。表1? 正常对照组和GHb升高组的LP(a)分布特点(略)
1.1.2? 正常对照组:选择健康体检中无糖尿病、无心血管疾病和肝肾等严重疾病者作对照组,共20例。年龄为38~78岁。男12例,女8例。
1.2? 方法:所有对象均在早晨空腹静脉采血。
1.2.1? HbA1c采用胶乳凝集反应法,上海申索佑福HbA1c试剂盒。采用抗凝血,标本2000rpm 2min离心分离后,从血细胞层中取10ul,加入1ml溶血液溶血,溶血后样品在Beckman LX-20全自动生化分析仪上测定。
1.2.2? Lp(a)采用免疫浊度法,上海申峰生物试剂,血清标本直接上Beckman LX-20全自动生化分析仪测定
