射频消融治疗快速心律失常29例体会 周俊(新疆库车县人民医院 842000) 【摘要】目的 探讨基层医院导管射频消融术(RFCA)治疗快速心律失常应注意 的事项。方法 对木院自2011年9月?2012年7月所作的14例RFCA和作者2008 年7月?12月在新疆维吾尔自治区中医医院进修时所作的15例RFCA手术情况进 行分析。结果29例患者中，其中12例患者为房室结双经路，6例为左侧旁路 (4例为左侧游离壁旁路、2例为左后间隔旁路)，5例室性早博(右室流出道室 早3例、左室游离壁室早1例)，2例为右室流出道室速，2例房扑，2例右侧旁 路(右侧游离壁)。结论 导管射频消融术(RFCA)是根冶快速心律失常有效的 新技术，其冶疗效果可靠、几乎无创。术前应完善相关检查，特别是要详细阅读 常规心电图和24小时动状心电图；并尽可能的行食道心房调搏术(TEAP)；熟练 掌握右心房解剖的左手模式；扎实的心脏电牛理基础、熟练的心导管操作技术及 术后的仔细观察是提高手术成功率和减少并发症的关键。 【关健词】基层医院快速心律失常导管射频消融体会 【中图分类号】R541.7 【文献标识码】A 【文章编号】2095-1752 (2013) 25-0071-02 Analysis of Radiofrequency ablation treatment of Rapid arrhythmias 29 Cases Zhou Jun (Kuche peoplersquo;s Hospital Xinjiang 842000) 【Abstract 】 Objective: To discuss the matters which should be paid attention to radiofrequency catheter ablation (RFCA) treatment of rapid arrhythmias in basic hospital. Methods: To analyze the surgical situation of 14 cases with the RFCA in our hospital from septemberer 2011 to July 2012 and 15 cases in Chinese Medicine Hospital of Xinjiang Uygur Autonomous Region from 2008 to 2012 in July. Results:For 29 patients, 12 patients shows atrioventricular nodal,6 cases shows left bypass (4 cases of left free wall bypass, 2 cases of left posterior interval bypass), 5 cases of room premature beat (right ventricular outflow tract as early as 3 cases, the left ventricular free wall of the chamber earlier cases), 2 cases of right ventricular outflow tract ventricular tachycardia, 2 cases of atrial flutter, the right of the two cases bypass (right free wall) Conclusion: Radiofrequency catheter ablation (RFCA) is a root rule of tachyarrhythmia new technology, as well as its effet is reliable cure and almost norninvasive? There are some key matters should be paid attention to in order to improve the success rate of srugery and reduce coplications, such as it is necessary toimprove the re leva nt checks before surgery, especially to read the ECG and 24-hour moving like ECG, possible line transesophageal atrial pacing surgery (TEAP),
