61-year-old male carpenter presented with ten months’
0 Able to carry out all normal activity without restriction
history of dyspepsia. More recently he had developed
1 Restricted in physically strenuous activity but ambulatory
Aepigastric pain and had lost four kilograms in weight and able to carry out light work
because of anorexia. He was otherwise asymptomatic and 2 Ambulatory and capable of all self care but unable to carry
in particular there was no history of nausea, vomiting, out any work; up and about more than 50% of waking hours
abdominal distension, melaena, haematemesis, diarrhoea, 3 Capable only of limited self care; confi ned to bed or chair
constipation, night sweats, or fevers. There was no past more than 50% of waking hours
medical history of note, and he had a performance status of 4 Completely disabled; cannot carry out any self care; totally
zero (see Table 1). His only medication was lansoprazole. He
