甲状腺外科手术中保护甲状旁腺功能的临床分析 　　摘要：目的 研究分析甲状腺外科手术中保护甲状旁腺功能的临床效果。方法 选取我院2014年7月～2015年7月收治的甲状腺患者56例为研究对象。所有患者均采取甲状腺全切手术进行治疗。分析患者手术前后血钙与血清甲状旁腺激素水平的变化。结果 所有患者均顺利完成甲状腺全切手术；在手术结束后的1个月，检查患者血钙与血清甲状腺激素，并未发现1例患者出现永久性甲状腺旁功能低下，将术后1个月患者血清甲状腺旁激素水平与术前进行对比，并未发现异常情况，差异不具有统计学意义（P0.05）；且术后第5 d与术后第30 d，患者血清甲状腺旁激素水平与术前进行对比，并未发现异常情况，差异不具有统计学意义（P0.05）。结论 在甲状腺全切术中，增强辨识甲状腺解剖位置，保护血供，对减少术后甲状旁腺损伤具有重要意义，且还可预防并发症的产生。 　　关键词：甲状腺外科手术；甲状旁腺功能；临床分析 　　Abstract：Objective To study the clinical effect of protecting parathyroid gland function in thyroid surgery. Methods 56 cases of thyroid patients admitted to our hospital from July 2014 to July 2015 were selected as study subjects. All patients were treated by total thyroidectomy. Analysis of the changes of serum calcium and serum parathyroid hormone levels in patients before and after surgery. Results All patients were successfully completed total thyroidectomy； in a month after the end of surgery， check the serum calcium and serum thyroid hormones， did not find cases of permanent thyroid dysfunction， postoperative one month of serum thyroid hormone levels and by preoperative comparison， found no abnormalities， the difference was not statistically significant （P0.05）； and the 5D and postoperative 30d， serum thyroid hormone levels and by preoperative contrast， found no abnormal situation， the difference was not statistically significant （P 0.05）. Conclusion It is great significance to enhance the identification of the anatomical position of the thyroid gland and protect the blood supply， and also can prevent the complications. 　　Key words：Thyroid surgery；Parathyroid function；Clinical analysis 　　手术治疗良恶性甲状腺疾病，全切手术以其治疗彻底、复发率低等特点被广泛应用。但是甲状旁腺的解剖位置比较特殊、形态较为?驮印⑹?量不定，增加了手术操作的难度，且术后还容易引发各类并发症，甲状腺功能低下就是一种常见的并发症。从临床报道中了解到，甲状腺功能低下病发概率在2%～33%[1]。对于永久性甲状腺功能低下，临床并无特殊的治疗方法，患者唯有长期服用药物。本文选取我院2014年7月～2015年7月收治的甲状腺患者56例为研究对象。研究分析甲状腺外科手术中保护甲状旁腺功能的临床效果。 　　1 资料与方法 　　1.1一般资料 选取我院2014年7月～2015年7月收治的甲状腺患者56例为研究对象。所有患者前来就诊时均发现颈部肿物，经过甲状腺彩超、CT检查与术后病理确认均为甲状腺。男34例，女22例，患者年龄为30～78岁，平均
