兔脓毒症早期心肌功能变化.PDF

维普资讯 中国危重病急救医学 2004年 6月第 16卷第 6期 ChinCritCareMed，June2004，Vo1．16，No．6 · 355 · · 论 著 · 兔脓毒症早期心肌功能的变化 潘小进 孙华 【摘要】 目的 研究脓毒症早期心肌功能变化的意义。方法 2O只兔随机分成两组：对照组 (一10)行 盲肠探查术；脓毒症组 (一1O)行盲肠结扎加穿孔术 (CLP)制备脓毒症模型。两组均经右侧颈 内动脉插管至 左心室，每隔 1h测量 1次左室收缩峰压(LVSP)、左室压力最大上升速率 (+dp／dtmax)、左室压力最大下降 速率 (一dp／dtmax)；并采血检测肌钙蛋 白I(TnI)，设术后 0、1、2、3和 4共 5个时间点。结果 脓毒症组动物 CLP术后 1h即出现 LVSP、+dp／dtmax、一dp／dtmax较基础值均明显下降(P均O．05)，且随着时间的发 展有进一步下降趋势；血清 TnI水平均明显升高 (P均O．05)，且随着时间的发展进一步升高。而对照组无明 显变化 。结论 兔脓毒症早期即有心肌损害，表现为心肌收缩和舒张功能均下降。 【关键词】 脓毒症； 心肌损害； 心肌功能； 肌钙蛋 白I 中图分类号：R631；R363 文献标识码 ：A 文章编号：1003—0603(2004)06—0355—04 Alterations in myocardialfunction in early stage ofsepsis in rabbits PAN Xiao —fin ，SUN Hua． IntensiveCareUnit，TheSecondAffiliatedHospitalofNantongMedicalCollege，Nantong226001， Jiangsu，China [Abstract] Objective Tostudythealterationsinmyocardia1functioninearlystageofsepsis．Methods Twentyrabbitswererandomlydividedintotwogroups．Inthecontrolgroup (，2— 1O)onlylaparotomywas done，and in thesepsisgroup theanimalsreceived cecalligation and puncture (CLP)．Inboth groupsleft ventricularcatheter wasplaced via rightinternalcarotid artery．Leftventricularsystolic peak pressure (LVSP)，maximalpositivechangeinfillingpressureversustime(+dp／dtmax)，maximalnegativechangein fillingpressureversustime (一dp／dtmax)weremonitored，andserum troponinI(TnI)wasmeasuredper hourforfivetimes(0，1，2，3，4hoursafteroperation)．Results Comparedto thebasiclevels。LVSP， +dp／dtmaxand—dp／dtmaxdecreasedsignificantlyanhourafterCLPinsepsisgroup(allP0．05)，with thetendencyofdecreasewithelapseoftime．Serum TnIincreasedsignificantlyanhourafterCLP insepsis group，andcontinuedtOincreasewiththepassageoftime．Incontrast，nosignificantchangewasobservedin controlgroup．Conclusion Cardiacmuscleisinjured，andmyocardialsystolicanddiastolicfuncti
