不同年龄段H型高血压合并慢性心力衰竭患者血清Hcy合成相关酶MTHFR、CBS及MTR表达研究   [摘要] 目的 研究不同年龄段H型高血压合并慢性心力衰竭患者血液中同型半胱氨酸(Hcy)合成关键酶甲基四氢叶酸还原酶(MTHFR)、胱硫醚合成酶(CBS)和蛋氨酸合成酶(MTR)的表达特点。 方法 2012年1月~2013年9月云南省第三人民医院心内科收治H型高血压合并慢性心衰患者64例,根据年龄分为三组:70岁为D组(13例);另选取30例体检健康人为正常对照组(A组)。采用ELISA测定炎性因子、总抗氧化能力(T-AOC)和同型半胱氨酸(Hcy)含量,Western blot及RT-PCR半定量检测全血中MTHFR、CBS和MTR mRNA及蛋白表达水平。 结果 B组、C组及D组患者的血管紧张素Ⅱ(Ang Ⅱ)、内皮素-1(ET-1)、肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)、丙二醛(MDA)和Hcy明显高于A组,差异有统计学意义(P 0.05),D组CBS表达明显低于A组,差异有统计学意义(P 0.05),D组MTHFR、MTR、CBS表达均明显低于B组,差异有统计学意义(P 0.05)。 结论 H型高血压合并慢性心衰患者血清中Hcy含量与年龄有关,这可能是Hcy合成相关酶MTHFR及MTR表达水平降低造成。   [关键词] 慢性心力衰竭;同型半胱氨酸;亚甲基四氢叶酸还原酶;胱硫醚合成酶;蛋氨酸合成酶   [中图分类号] R541.6 [文献标识码] A [文章编号] 1673-7210(2016)04(b)-0060-04   [Abstract] Objective To study the expression of serum Hcy synthesis key enzyme MTHFR, CBS and MTR in H-type hypertension patients with chronic heart failure. Methods From January 2012 to September 2013, in the Third Peoples Hospital of Yunnan Province, 64 H-type hypertension patients with chronic heart failure were selected, According to the patients ages, they were divided into three groups, 70 years were as group D (13 cases) over, 30 healthy people in physical examination were selected as group A. The expression of inflammatory cytokines, total antioxidant capacity were detected by Elisa, mRNA and protein of MTHFR, CBS and MTR were detected by Western blot and RT-PCR. Results The levels of Ang Ⅱ, ET-1, TNF-α, IL-1β, MDA and Hcy in group B, group C and group D were significantly higher than those in group A, the differences were statistically significant (P   [Key words] Chronic heart failure; Hcy; MTHFR; CBS; MTR   心力衰竭(简称“心衰”)临床中又称之为心功能不全[1],主要是指血液静脉回流时,心脏由于一些已知或未知的原因导致其收缩和/或舒张功能发生障碍,收缩或舒张功能的障碍可导致其心排血量不足,排血量的不足又会导致身体代谢能量供应不足,最终造成了本病发生与发展[2]。H型高血压合并心衰是指伴有高同型半胱氨酸血症的原发性高血压病患者同时合并慢性心衰的症状[3],近年研究表明,H型高血压可通过规范降压治疗及补充叶酸和维生素B12降低高同型半胱氨酸从而改善预后[4],但是研究[5]也发现有些患者通过上述治疗效果并不理想,本文通过研究不同年龄段H型高血压合并慢性心衰患者血液中同型半胱氨酸(Hcy)合成关键酶亚甲基四氢叶酸还原酶(MTHFR)、胱硫醚合成酶(CBS)以及蛋氨酸合成酶(MTR)的表达特点,为本病的针对性治疗提供一定的参考。
