278 Chin Otorhinolaryngol J Integ Med 2008,Vol.16.No.4 放射影像技术在小儿气管、支气管异物诊断 中的应用价值 覃纲1 杨洪斌2 田蓉3 叶惠平3 梁传余3 王力红3 刘世喜3 【摘要】目的 探讨影像学技术在小儿气管、支气管异物诊断中的应用价值。方法 对98例气管、支气管异物患儿的临床经 过及相关影像学资料进行回顾性分析，总结其影像资料对小儿气管、支气管异物的诊断价值。结果 98例患儿中，74例进行了术 前X线胸片及透视检查，表现异常者46例；行普通CT检查15例，表现异常者13例，其中5例直接显示异物；行螺旋CT并三维重 建10例，均表现异常，且有8例直接显示异物。除l例因术前窒息死亡外，其余病例均成功进行了支气管镜检查及异物取出术；其 中94例一次性取出异物，2例于术前自行咳出异物，l例行支气管镜检查未发现异物。结论 影像学检查对气管、支气管异物具有 辅助诊断价值，但影像学检查结果不能作为是否进行支气管镜检查的绝对依据。 【关键词】影像学技术：气管支气管异物；儿章；诊断 Imaging evaluation for the diagnosis of tracheobronchial foreign bodies among children cases QIN Gang，YANG Hongbin，TIAN Rong，YE Huiping,LIANG Chuanyu，WANG Llhong,LIU Shixi Dept.of Otolaryngology -Head and Neck Surgery,the Affiliated Hospital Of Luzhou Medical College,Sichuan,646000,China 【Abstract】Objective To explore the feasibility and clinical implications of radiation imaging techniques in the diagnosis of tracheobronchial foreign bodies among children cases based on a clinical trial. Methods A retrospective study was carried out among 98 children cases with tracheobronchial foreign bodies treated in our Hospital from Sep. 2005 to Jun. 2006. Their clinicaI courses in hospitalization and imaging data collected during this period were carefully analyzed to value the evaluation of radiation imaging techniques in the diagnosis of such a condition. Results Among these 98 cases, 74 were examined preoperatively with chest radiography or fluoroscopy. with abnormal findings found in 46 cases, 1 5 were given routine CT scanning. with abnormal images shown in 1 3 cases and furthermore, with the foreign bodies directly displayed on the CT images in 5 out of these 1 3 cases, and spiral CT with 3-D reconstruction
