Filling requirements 1. This form shall be filled in TWO COPIES by permanent writing ink and shall not be altered. All information stated herein shall be true, complete, accurate and in required form. Once altered, the relevant content shall accompany with signature of the legal representative of the project or the official seal of legal representative. This form shall be submitted to the Suzhou Industrial Park of one copy, and the other copy shall be duly kept by your company. 2. All information in this form filed shall be reviewed by the legal representative of the project and once approved, his/her signature or official seal shall be attached. Where an appointed person is to sign on this form, power of attorney of the legal representative of the project shall be attached. 3. FOR PROJECTS IN SURROUNDING TOWNS AND DISTRICTS, THIS FORM SHALL BE FIRSTLY SUBMITTED FOR APPROVING BY THE LOCAL ENVIRONMENT PROTECTION OFFICE OF TOWN OF SUCH PROJECT BEFORE SUBMITTED TO THE ENVIRONMENTAL PROTECTION BUREAU OF SUZHOU INDUSTRIAL PARK. II. Attachments 1. Project location map clearly stating the project site shall be attached; for projects in the China-Singapore Cooperation Zone, the original location map is available from One-stop service center, for projects in surrounding towns and districts, such location map is available from the local environment protection office of town. 2.
