目 的
对国内外有关人工髋关节置换治疗髋关节疾病的研究文献进行分析,评价髋关节表面置换术与常规全髋关节置换术的临床效果,为临床髋关节置换手术术式选择提供理论依据。
方 法
计算机检索1990年1月至2012年9月间MEDLINE、EBSCO、EMbase、PubMed、CHKD、CNKI、CBM、维普、万方数据库发表的有关髋关节表面置换与全髋关节置换临床效果比较的文献资料。主要是对术后髋关节功能的比较,由2位评价者按照纳入和排除标准独立选择文献、提取资料、评价质量,然后采用stata10.0软件对纳入研究进行Meta 分析。
结 果
最终纳入八篇文献,共计986例髋关节置换术患者,其中表面置换术组患者460例,全髋关节置换术组患者526例。Meta分析发现,与常规全髋关节置换术相比,髋关节表面置换术后髋关节Harris功能评分差异有统计学意义(P=0.040)。
结 论
与THA相比,HRS后患者术后功能更佳,因此应根据临床手术适应症,适当选择髋关节表面置换手术。

The short-term clinical efficacy of hip resurfacing arthroplasty compared to conventional total hip arthroplasty: a Meta-analysis
Objective
Through the analysis about the hip arthroplasty therapy hip disease research literature at home and abroad, evaluate clinical effect of both hip resurfacing arthroplasty and conventional total hip arthroplasty for clinical hip arthroplasty surgery procedures provide theoretical basis for choice.
Methods
This study retrieved the literatures about hip resurfacing with total hip arthroplasty postoperative hip function comparison of clinical effects from MEDLINE, EMbase, EBSCO, PubMed, CHKD, CNKI, CBM, VIP, wanfang database between 1990 and sep 2012. The two evaluators choose literature, extract data and evaluate quality according to into and exclusion standard. Meta-analysis were performed by Stata 10.0. Result 9 literatures were selected in the following meta-analysis, including 986 cases of patients with hip arthroplasty. Among these patients, 460 were included in hip resurfacing arthroplasty group, and 526 were in conventional total hip arthroplasty group. Postoperative Harris hip score was statistically significant between conventional total hip replacement surgery and hip resurfacing.
Conclusion
Hip resurfacing arthroplasty is better than total hip arthroplasty in postoperative Harris hip function score, and should therefore be properly selected based on clinical indication for surgery.
