非脱垂阴式子宫切除术与经腹腔子宫切除临床疗效分析.pdf

医学临床研究 2008年6月 第25卷 第6期 J Clin Res，Jun．2 ，V_ !No ． LJ 乍J菩 ． 非脱垂阴式子 宫切除术与经腹腔子宫切除临床疗效分析 张兰珍 (湖南省桃源县人民医院，湖南 桃源 415700) [摘要]【目的】比较非脱垂阴式子宫切除术与经腹子宫切除的临床疗效。【方法】回顾性分析本院2001 年2月至2007年7月子宫切除治疗的218例妇科良性肿瘤的临床资料，根据手术方式的不同分为：非脱垂阴 式子宫切除组(A组)和经腹子宫切除术(B组)，比较两组手术时间(d)、术中出血量(mL)、术后下床活动时间 (h)、住院天数(d)、肛门排气时间(h)、并发症的发生率及住院费用及术后3个月随访情况。【结果】A组手术 时间，肛门排气时间均低B组，差异有显著性(P d0．o5)。术中出血量，术中、术后并发症发生率及术后3个 月随访阴道残端愈合等与B组比较差异无显著性(P0．05)。【结论】非脱垂经阴道子宫切除术创伤小，术 后恢复快，符合微创的原则，不增加手术难度不大，值得在基层医院推广应用。 [关键词] 子宫切除术 TAnalysis on Clinical Efficiacy of Transvaginai Hysterectomy and Transabdominai Hysterecto— my ZHANG Lan—zhen(the Peoples Hospital of Taoyuan County，Hunan 415700，China) [Abstraet][Objective]To evaluate the clinical applications of transvaginal hysterectomy and transabdomi— nal hysterectomy．[MethodslClinical data of 218 patients with benign uterine diseases treated by transvaginal hysterectomy and transabdominal hysterectomy were retrospectively reviewed．The duration of operation， bleeding volume during operation，hospital stay after operation，the time of bowel function recurrence，the du— ration of operat Lon，bleeding volume during operation，hospital stay after operation，the time of bowel function recurrence，intt a—or post_operative complications and cost of hospita1ization were compared by retrospective study．[Results]There were significant differences in the duration of operation，bleeding volume during opera— tion，hospital stay after operation，the time of bowel function recurrence and the cost of hosDitalization．There was no statistical significant difference in occurrence of intra—or post-operative complications and the blood loss in operation between transvaginal hysterectomy and transabdominal hysterectomy．[Conclusion]Transvaginal hysterectomy has advantages of minimal invasion and rapid recovery，but depends on the operators techno
