并行循环下不停跳冠状动脉旁路移植术在 冠心病患者中的应用 Coronary artery bypass surgery with on-pump beating-heart technique 沈立 杨文钢 黄日太 薛松 作者单位：200127 上海市，上海交通大学医学院附属仁济医院心血管外科 通讯作者：薛松，E-mail：xuesong64@163.com 【摘要】 目的 评价并行体外循环下不停跳冠状动脉旁路移植术在冠心病患者中的应用价值。方法 回顾性分析2008年1月至2013年1月于仁济医院心血管外科行并行体外循环下不停跳冠脉旁路移植术（OnPBH-CABG）患者42例（男性30例、女性12例），选取非体外循环冠脉搭桥术（OPCAB）患者62例（男性46例、女性16例）对照，比较术前及围手术期临床资料。结果 并行循环组术后早期临床效果良好，呼吸机辅助时间［（21.73±17.69）h］、重症监护病房停留时间［（4.64±2.11）d］、院内死亡率［（2.38%］及术后各类并发症等指标与OPCAB组比较均未见统计学差异。结论 并行体外循环下不停跳冠状动脉旁路移植术是一种安全、有效的手术方式。 【关键词】 冠状动脉旁路移植术； 心脏不停跳； 体外循环 Coronary artery bypass surgery with on-pump beating-heart technique SHEN Li， YANG Wen-gang， HUANG Ri-tai， et al. Results Patients underwent On-pump beating heart CABG had favorable early clinical outcome. No significant differences were found in the ICU days（4.64±2.11）d in OnPBH group， intubation time（21.73±17.69）h in OnPBH group， hospital mortality（2.38% in OnPBH group） or other postoperative data complications included between the two groups. Conclusion On-pump beating heart CABG can be performed safely and effectively. 【Key words】 Coronary artery bypass grafting； Beating-heart； Extra corporeal circulation 冠心病的外科治疗一直是心血管病研究的重要课题。近年来，非体外循环冠状动脉旁路移植术（off-pump coronary artery bypass，OPCAB）得到广泛应用，其避免了体外循环带来的一系列生理和病理紊乱，但术中因心脏搬动一些患者可能出现血流动力学不稳的情况，故该术式也有一定局限。并行循环下不停跳冠脉旁路移植术（on-pump beating-heart CABG，OnPBH－CABG）可避免OPCAB手术中因为搬动固定心脏引起的血流动力学不稳带来的危害[1]。本研究就两种手术进行比较并报道如下。 1 资料与方法
