中国普外基础与临床杂志 2017年6月第24卷第6期 • 765 •   ·综 述· 食管胃结合部腺癌的研究进展 1 2 1 1 1 杨勇 ，李卫平，熊辉，廖宗源，张江南 1. 南昌大学第一附属医院胃肠外科（南昌  330006 ） 2. 太仓市第一人民医院胃肠外科（江苏太仓  215400 ） 【摘要】   目的    总结近年来食管胃结合部腺癌（AEG ）的研究进展，以提高对该病的进一步认识。方法    对 国内外关于 AEG 的文献进行综述。结果    Siewert 分型是当前较为公认的 AEG 分型，其将 AEG 分为 Ⅰ 型、Ⅱ 型 及 Ⅲ 型。胃食管反流病、Barrett 食管以及幽门螺杆菌（HP ）感染均可能是 AEG 发病的重要因素。Siewert Ⅰ 型 AEG 患者首选经胸径路手术，Siewert Ⅱ、Ⅲ 型 AEG 患者推荐经腹全胃切除及经扩大食管裂孔远端食管部分切 除。此外，内镜微创技术、新辅助治疗、靶向治疗、多学科团队（MDT ）等治疗方法或理念越来越受到重视。 结论    不同 Siewert 分型的 AEG 的手术方式和切除范围不同，应全面评估患者病情，选择个体化的最佳治疗方 案，以确保疗效。 【关键词】  食管胃结合部腺癌；Siewert 分型；淋巴结转移；外科手术；化疗 Research progress of adenocarcinoma of esophago-gastric junction 1 2 1 1 1 YANG Yong , LI Weiping , XIONG Hui , LIAO Zongyuan , ZHANG Jiangnan 1. Method    The literatures about application and research progress of AEG were researched and reviewed. Results    Siewert classification was a widely recognized classification for AEG, according to this classification, AEG was divided into type Ⅰ, Ⅱ, and Ⅲ. Gastroesophageal reflux, Barrett’ sesophagus, and the infection of Helicobacter pylori may be the important pathogenic factors.
