无创通气治疗对慢阻肺合并Ⅱ型呼吸衰竭患者临床疗效观察 　　[摘要] 目的 探讨无创通气治疗对慢阻肺合并Ⅱ型呼吸衰竭患者的临床疗效情况。方法 整群选取海南省万宁市人民医院2014年12月―2015年12月收治的慢阻肺合并Ⅱ型呼吸衰竭患者51例的临床资料，依据患者治疗方式不同进行分组，常规治疗组21例和无创通气治疗组30例。观察两组慢阻肺合并Ⅱ型呼吸衰竭患者治疗前后FEV1、FVC、FEV1%情况。结果 两组慢阻肺合并Ⅱ型呼吸衰竭患者治疗前FEV1（1.40±0.22） L和（1.42±0.23） L、FEV1/FVC（43.3±5.2）和（44.4±5.3）、FEV1%（47.6±3.5）%和（48.3±3.6）%，差异无统计学意义（P0.05），无创通气治疗组治疗后FEV1（2.88±0.42）L、FEV1/FVC（78.3±4.1）、FEV1%（75.4±4.3）%均优于常规治疗组（1.95±0.36）L、（66.1±4.0）、（64.2±4.1）%，差异有统计学意义（P0.05）。结论 无创通气治疗慢阻肺合并Ⅱ型呼吸衰竭患者临床症状改善明显，疗效良好，值得临床推广应用。 　　[关键词] 无创通气；慢阻肺合并Ⅱ型呼吸衰竭；疗效 　　[中图分类号] R563.9 [文献标识码] A [文章编号] 1674-0742（2016）04（a）-0022-03 　　[Abstract] Objective To discuss the clinical curative effect of noninvasive ventilation in treatment of patients with chronic obstructive pulmonary disease combined with type Ⅱ respiratory failure. Methods 51 cases of patients with chronic obstructive pulmonary disease combined with type Ⅱ respiratory failure admitted and treated in our hospital from December 2014 to December 2015 were selected and divided into the conventional treatment group with 21 cases and the noninvasive ventilation treatment group with 30 cases， and the FEV1， FVC and FEV1% situations of the patients before and after treatment were observed. Results There was no obvious difference in the FEV1， FEV1/FVC and FEV1% between the two groups before treatment， [（1.40±0.22） L， （43.3±5.2）， （47.6±3.5）% vs （1.42±0.23） L， （44.4±5.3）， （48.3±3.6）%]，（P0.05）， the FEV1， FEV1/FVC and FEV1% in the noninvasive ventilation treatment group were better than those in the conventional group after treatment， [（2.88±0.42） L， （78.3±4.1）， （75.4±4.3）% vs （1.95±0.36） L， （66.1±4.0）， （64.2±4.1）%]， and all differences had statistical significance（P0.05）. Conclusion The clinical symptom improvement of noninvasive ventilation in treatment of patients with chronic obstructive pulmonary disease combined with type Ⅱ respiratory failure is obvious， and the curative effect is good， which is worth clinical promotion and application. 　　[Key words] Noninv
