子宫腺肌病中缺氧诱导因子―1表达及其临床意义 　　【摘要】 目的：探讨子宫腺肌病患者在位和异位内膜组织中缺氧诱导因子-1蛋白的表达及其临床意义。方法：采用免疫组化方法检测子宫腺肌病患者的在位和异位内膜组织各60例，并将其与正常子宫内膜30例的阳性率进行统计学分析。结果：HIF-1表达中，子宫腺肌病内膜、肌层的表达大于正常子宫内膜及肌层。子宫腺肌病中异位和在位内膜中的表达为86.67%和65.00%，相比正常子宫内膜33.33%，差异有统计学意义（P0.05）。异位内膜表达也高于在位内膜，比较差异有统计学意义（P0.05）。结论：子宫腺肌病的发生、发展与HIF-1关系密切，可促进子宫腺肌病细胞的生长、血管形成。 　　【关键词】 子宫腺肌病； 缺氧诱导因子-1； 免疫组化 　　【Abstract】 Objective：To explore the clinical significance and expression of Hypoxia inducible factor-1 in adenomyosis.Mehtod：Immunohistochemical method was used to detect the each 60 cases of eutopic and ectopic endometrial tissue with adenomyosis， and the positive rate was compared with that of normal endometrium in 30 cases.Result：The expression of the muscular layer and endometrium of adenomyosis in HIF-1 were greater than that of normal endometrium and muscular layer；expression in ectopic and eutopic endometrium of adenomyosis were respectively 86.67% and 65.00%， compared with normal endometrium of 33.33%，there were statistically significant differences（P0.05）.The expression of ectopic endometrium was higher than that in eutopic endometrium，the difference was statistically significant（P0.05）.Conclusion：The occurrence and development of adenomyosis is closely related to HIF-1，which can promote the growth and angiogenesis of adenomyosis. 　　【Key words】 Adenomyosis； Hypoxia inducible factor-1； Immunohistochemical 　　First-author’s address：People’s Hospital of Yingde City in Guangdong Province，Yingde 513000，China 　　doi：10.3969/j.issn.1674-4985.2017.07.008 　　子?m腺肌病是指子宫内膜腺体及其周围间质慢性侵袭进入子宫肌层，进行性生长而产生的病变，发病机制不清，主要表现为继发性进行性加重的痛经、月经量增多、不孕不育等，已成为危害广大妇女身心健康的重要妇科疾病，药物治疗以及保守手术治疗效果差，尤其对渴望生育的妇女临床治疗非常棘手。研究认为缺氧诱导因子-1（HIF-1）为缺氧状态下可调控新生血管生成，本研究旨在通过临床资料分析，免疫组化检测HIF-1在子宫腺肌病中的表达情况并探讨临床意义，现将结果报道如下。 　　1 资料与方法 　　1.1 一般资料 收集本科2012年1月-2015年1月诊断明确的子宫腺肌病患者异位、在位内膜组织各60例，年龄40～52岁，平均（46.2±3.1）岁；其中增生期患者38例，分泌期22例；月经起始时间、周期异常46例（76.67%）；痛经病史54例（90.00%）。同时选取同年份在本科因非子宫腺肌病疾病前来门诊或病房行诊刮术的正常子宫内膜30例作为对照组，年龄26～58岁，平均（45.7±3.0）岁，刮诊术后内膜组织经病理诊断考虑为正常子宫内膜，月经起始时间、周期异常2
