中国药物应用与监测 2013 年10月第10卷第5期 Chinese Journal of Drug Application and Monitoring, Vol.10, No.5, October 2013 CYP2C19 基因多态性与氯吡格雷抗凝作用的相关性研究 1 2 2 2 2 （1.河北北方学院，河北 张家口 075000 ；2. 解放军309 医院药剂科，北京 蔡泓敏 ，陈　珲 ，赵冠人 ，李　素 ，冯端浩 100091 ） [摘要] 目的：探讨细胞色素P450（CYP ）2C19 基因多态性与氯吡格雷抗凝作用的关系，为氯吡格雷个体化用药提供参考。 方法：选取诊断为急性冠脉综合征并接受经皮冠状动脉介入治疗术（PCI ）的患者，采取血标本并提取外周血基因组DNA ，将 提取后的总DNA 进行PCR 扩增，扩增产物经基因芯片杂交法确定基因型，根据不同的基因型对患者进行分组。176例入选 病例共分为3 组，A 组：快代谢基因组（CYP2C19*1/*1 ）；B 组：中等代谢基因组（CYP2C19*1/*2、CYP2C19*1/*3 ）；C 组：慢代 谢基因组（CYP2C19*2/*2、CYP2C19*2/*3 或CYP2C19*3/*3 ），测定各组ADP 抑制率。应用SPSS 13.0 软件对数据进行统计 P 分析。结果：176例入选病例三组之间ADP 抑制率差异无统计学意义（ 0.05 ），127例首次行PCI 患者中C 组与B 组和A 组 P P 相比，ADP 抑制率差异有统计学意义（ 0.05 和 0.01 ）。结论：首次经皮冠状动脉介入治疗术后CYP2C19 慢代谢基因型 患者常规使用氯吡格雷疗效较差。 [关键词]　CYP2C19；基因多态性；氯吡格雷；ADP 抑制率 [ 中图分类号]　R973+.2　　 [ 文献标识码]　A　 [ 文章编号]　1672 – 8157(2013)05 – 0254 – 04 Correlation study of gene polymorphism of CYP2C19 and anticoagulation effect of clopidogrel 1 2 2 2 2 (1.
