非小细胞肺癌纵膈淋巴结分期的进展 万人安 尹逊亮 综述 魏煜程 审校 作者单位:266000青岛,青岛大学医学院附属医院胸外科(通讯作者:魏煜程,E-mail:weiyuchengchest@163.com) 【摘要】非小细胞肺癌 non small cell lung cancer, NSCLC 纵膈淋巴结分期十分重要,它可以评估疾病进展程度,指导临床治疗方案并决定患者的预后。无创性影像学分期效果整体较差,有创性外科分期方法具有更高的诊断效能:纵膈镜目前作为纵膈淋巴结分期诊断的金标准;超声内镜针吸活检术创伤更小并对纵膈淋巴结诊断具有高灵敏性;胸腔镜应用于纵膈淋巴结的分期同样安全有效;IIIA期肺癌诱导治疗后的纵膈淋巴结再分期技术要求较高,首选有创性分期方法。早期肺癌的术中前哨淋巴结活检作为一个新兴技术正迅速发展并在临床试验中取得了良好的诊断效能,但仍需进一步研究论证。 【关键词】肺癌 纵膈 淋巴结 分期 Progress of mediastinal lymph node staging for non small lung cancer Renan WAN, Xunliang YIN, Yucheng WEI Department of thoracic surgery, The Affiliated Hospital of Qiingdao University, Qingdao266000, China Corresponding author: Yucheng WEI, E-mail:weiyuchengchest@163.com 【Abstract】 For patients with non-small-cell lung cancer NSCLC , mediastinal staging is very important as it provides accurate information on the extent of the disease, guides the choice of treatment and determines the patients prognosis.Non invasive imaging techniques are not sufficiently reliable in many situations, making invasive staging tests an important part of appropriate staging: Mediastinoscopy remains the “gold standard” in mediastinal staging; Endoscopic ultrasound- needle aspiration techniques are less invasive and has a high sensitivity; Video-assisted thoracoscopic surgery used in mediastinal lymph node staging also is safe and effective; Mediastinal re-staging after induction therapy for Stage IIIA NSCLC is request higher, invasive staging techniques as the first choice. Sentinel lymph node biopsy as a new technology is developed rapidly and achieved good results in clinical trials , but still need to be further studied. 【Key words】 Lung cancer; Mediastinum; Lymph node; Staging 肺癌是发病率和死亡率增长最快,对人群健康和生命威胁最大的恶性肿瘤之一,其中非小细胞肺癌约占80%。对NSCLC患者进行纵膈淋巴结分期诊断非常重要,它可以评估疾病进展程度,指导临床治疗方案并决定患者预后情况。目前对NSCLC纵膈淋巴结分期诊断的方法,大体可分为无创性影像学分期方法和有创性外科分期方法以及术中前哨淋巴结活检术。现将各种分期方法的应用进展进行综述。 1无创性影像学分期 1.1计算机断层成像(Computed tomography, CT) CT于20世纪80年代广泛应用到肺癌的影像学检查中,以形态学来评价纵膈淋巴结的良恶性。美国胸内科医师协会 ACCP 指南指出CT的敏感性、特异性、阳性似然
