[Key words] PIVAS;Process;Error;Notes 静脉药物配置不仅是医院实施输液配置方式改革的一个平台,也是临床药师积极探索合理用药的重要舞台。2002年1月,卫生部公布实施的《医疗机构药事管理暂行规定》,文中明确指出:“医疗机构要根据临床需要逐步建立静脉药物配置中心,实行集中配置和供应”。逐步建立实行静脉药物配置中心已经成为医院药学工作的重点,也是衡量医院综合实力的重要准则。虽然卫生部已公布了相关规定和准则。但是由于我国的静脉配制中心起步较晚,如何建立一套最优化的的流程,如何注意静脉药物配置工作流程各个注意事项、管理模式,成为广大药学工作者关心的问题。2006年7月,我院建立了PIVAS,经过近3年摸索和磨和,逐渐走上了正轨,建立了一套较为成熟的工作流程。 一般来讲,PIVAS的工作流程一般都为:临床医生开具静脉输液医嘱→护士整理医嘱→药师审核医嘱及打印医嘱→打印标签并按批次分标签→分批排药贴标签核对→传递入抗生素细胞毒药物配置间及营养普通药物配置间混合调配→成品核对→成品包装→分科室置于密闭容器中→工人送至病区→楼层护士核对→给患者静脉滴注用药。不同的医院其静脉药物配置中心工作流程
