Introduction:

Thesurvivalofrandomflapsinplasticsurgeryislargelydeterminedbythebloodsupplytotheflap.Vascularendothelialgrowthfactor(VEGF)isakeyregulatorofangiogenesisandhasbeenshowntoimproveflapsurvivalinvariousanimalmodels.However,theoptimaldosageandadministrationrouteofpreoperativeVEGFinjectionforflapsurvivalremainunclear.ThisstudyaimstoinvestigatetheeffectofpreoperativelocalinjectionofVEGFonthesurvivalofrandomflapsinrats.

Objectives:

1.TodeterminetheoptimaldosageofVEGFforpreoperativelocalinjectionthatpromotesflapsurvivalinrats.

2.TodeterminetheoptimaltimeframeforVEGFinjectioninrelationtoflapsurgery.

3.ToassesstheeffectofpreoperativelocalinjectionofVEGFonvascularizationandtissueviabilityofrandomflapsinrats.

Methodology:

Forty-eightadultmaleWistarratswillberandomizedintofourgroups(n=12pergroup):control,low-dose(10μg),medium-dose(50μg),andhigh-dose(100μg)VEGFinjectiongroups.TheratsintheexperimentalgroupswillreceivelocalinjectionsofVEGFonthebackskinbeforerandomflapsurgery.Thecontrolgroupwillreceivesalineinjectionsinstead.Theflapsizewillbestandardizedto4x4cm,andtheflapswillbeelevatedonthesubfascialplane.Theviabilityoftheflapswillbeassessedbymeasuringthesurvivingflapareaondays2,7,14,and28postoperatively.Onday28,theflapswillbeharvestedforhistologicalanalysistoevaluatevasculardensityandtissueviability.

ExpectedOutcomes:

WeexpecttodemonstratethatpreoperativelocalinjectionofVEGFimprovesthesurvivalofrandomflapsinratsinadose-dependentmanner.Weanticipatethattheoptimaldosagewillbebetween50to100μg,withthebesttimingforinjectionbeing24hoursbeforet
